Cancellation Policy

 

Your appointments and well-being are very important to us. We understand that sometimes, unexpected changes in your schedule may occur. If you need to cancel your appointment, we respectfully request a 24-hour notice via email or phone.  A cancellation fee will be accessed on any and all reservations that are canceled without proper notice.

 

Please understand that appointment times are limited.  We ask you to be mindful that we require a 24-hour cancellation notice to give us enough time to notify the scheduled nurse for canceled appointments and other key staff members.  All appointments are subject to change due to availability, travel circumstances, government or health department regulations, and any other unforeseen events.

 

Our Cancellation Policy:

 

    • We require a 24-hour notice via email or phone to cancel a reservation without penalty.  To cancel your reservation, email hydrate@vivifyiv.com or call (602) 671-6645.

 

    • Any reservations that are canceled less than 24-hours from your scheduled appointment time will be partially refunded, less a non-refundable cancellation fee of $50.00.

 

    • If your appointment is re-booked within 7 days of the original appointment date, the cancellation fee will be waived.  In other words, if you move your appointment (within 7 days) instead of canceling it, there will be no penalty.
